Administrative support, mail, and travel must be required specifically by the proposed mini symposium, and must be clearly justified.
- Budget Justification. Clearly justify all costs fully.
- NIH BioSketch of Principal Investigator and Co-Investigator(s) (4 page maximum) Use Form "PHS 398/2590" from the NIH Grant Applications with other support pages. Include active, pending, and planned proposals. Include percent effort, total direct costs (current year) and potential overlap with the current proposal.

Department Chair/Unit Head should indicate support for the application with signature.

Does the proposed project include a sub-award?
Is the applicant PI appointed at an institution other than UCSF? If so, then any award, if funded by CTSI-SOS, will be a sub-award.
If you checked either of these boxes, your RAP application requires additional information to ensure timely set-up of your award.
Many of the agencies that make awards through RAP are funded by federally sponsored projects. Therefore, sub-awards on RAP projects must be approved by the parent funding agency in advance of the project start date. CTSI-SOS parent funding agencies require the following information:

- Project period
- Pilot Project Principal Investigator’s Name, Organization and Address
- What is the nature of the collaboration? If the sub-award is a foreign institution, will domestic researchers be traveling and conducting research at the foreign site?
- ESTIMATED TOTAL COSTS for the entire project period
- ESTIMATED CONTRIBUTION by sub-award institution (if applicable)
- Sub-award Investigator(s) Name, Organization, City, Country, Email, Phone
- Research Project Objectives
- Human Subject Involvement
- Assurance no. for the sub-award site
- Who are the participants for the interviews, surveys, and focus groups?
- What is the sample size?
